I welcome your thoughts on the following:

    - car was equipped with G9200-47162 inverter
    - while driving the car entered in the limp mode and I made it to the dealer; showed P0A94
    - the dealer contacted Toyota and told me the car had the inverter replaced through a recall and that they do not cover for another replacement (surprise to me because I am the 2nd owner and did not have that detail)
    - replaced myself the inverter with G9200-47220 (used bought on ebay)
    - the car started but stopped after 100 yrds and then, after 30 min, started and ran for 5 miles
    - the next day the car engine did not start anymore and it only goes to N, shows P0A78, P0A7A, and P0A94.

I appreciate any advice that would help me fix this car.

    Thank you in advance!

How long have you owned the car, what’s the mileage?
Do you know which recall that the dealer referenced?
Do some research on the 23TE03 customer care, it covers inverters for 20 years, depending on the codes showing.
Mine is in now awaiting the parts to get replaced, as the second owner, 176320 miles, and had not done any of the recent recall updates since I bought it used in 2017.

What device or program did you use to read the codes?

Do you have information on the freeze frame of the error codes?

    It would help more significantly.

Hello the car has 160k and I went to the dealer with the documentation for the 23TE03 program and according to Toyota my car is not covered anymore because the inverter was replaced through a recall by the previous owner. See below for the current errors. IMG_4024.jpeg IMG_4023.jpeg IMG_4022.jpeg

Might want to check what car an # G9200-47220 inverter fits, and not just from an ebay ad selling it.
    That's my first wild guess.

This number is suitable. But having bought an inverter on the Ebay platform there is no guarantee that this inverter is problematic and has not been removed earlier due to a similar error.

This is a multi-brand scanner MS309. Unfortunately, it will not help much in diagnosing a hybrid installation.
    https://www.autel.com/u/cms/www/202004/23032946p9xd.pdf

There are 23 error subcodes for the P0A78 error code, which indicate different problem areas. The testing method for different subcodes varies and is quite extensive.

Without these subcodes and a freeze frame of the error, you have only two options:
Find a special scanner that reads hybrid errors with subcodes and reads freeze frame data or change the inverter and other hybrid installation units one by one to restore functionality.

I know for a fact the inverter is the issue. Let's say I install a new matching inverter out of the box.
After I install the new part, do I have to perform any software update or software manipulation?

It is unlikely that you will be able to flash the inverter.
And there may be no need for this.
After installing the inverter correctly, start the car, reset the errors and this will be enough to drive.
If possible, use the Techstream program to find out what version of the firmware is in the inverter software and ask the dealer about the current version.

The reason I'm not posting more to this thread is that the US market vs the UK market differences.

    Here's a link to a US market TSB for a Gen 3 Prius V ( 2012 - 2014 )
Please note, I wouldn't recommend using this to diagnose your 2016 Prius V (uk model) or even a 16 V (us model ).
But it may give some hints to what you might see in the Toyota Repair Manual, if the repair manual is even available to the public in the UK , ( I read here some EU UK members don't have access )

anyways, keeping fingers crossed this may offer some hints.
