Simply for the options. They are all the same car with the same hybrid system. You need to determine the comfort level you desire.
I went for a IV so I could get the 3 door sks, Homelink transmitter and the heated seats with lumbar support. The leather was just a nice extra.
What others have said, it adds 3 door sks, leather, homelink with auto dimming mirror as well as the glass repellent windows on the front drivers and passenger doors. Only you can determine if you want those options and step up to a Prius IV...

I'd get the 3 and add leather, lumbar and heat and a homelink mirror aftermarket. I figure you save $1000 at least. Seats can be done very well for $1500, probably a lot less. Mirror is $100 or so and install a couple hundred max. It would be nice to have 3 door SKS but I'll live with pressing the button on the hatch.
