Over the weekend, I had the red triangle of death appear, alomg with (!), VSC, and the car with exclamation mark on the MFD; and yesterday I noticed the HV battery fan was blowing really loud. I cleaned the fan last night, but that didn't help. I also notoced while driving today that, according to the MFD Energy Monitor, the car never ran on just the battery, even when statting from a stop. Instead, it immediately went into the combo gas/electric power.

I got an OBD2 scanner today, and did an initial check with Dr Prius. All I did was turn on the car in the garage and let it run a short time to see what it showed. The specific codes I'm getting are P0A80, P3000, P3021, and P3022. Sitting in the garage, the car kept shifting between idling and charging the HV battery every 5-15 seconds. Just from this check, block 11 seems to be the bad one - it was measuring around 15.5V when idling vs the others around 16.9, and 17.7 when charging vs the others around 17.3. The big difference I see is with the internal resistance - Block 11 was up to 33 milli-ohms vs the others around 22.

The car is currently around 228k miles. The car has had poor luck with the HV battery recently - it was replaced around 2020/2021 at around 180k miles, when the car was still my parent's. The first replacement they got was bad, the second one worked fine. I then purchased the car from parents within a year. Then in July '22, I took in the car to get something else done, amd the battery failed again when they did the test drive afterwards. It is still droveable now, bit who knows for how much longer, given I have to drove around 100 miles a day for work.

I am assuming that I am going to have to replace the module at the very least, but is there anything else I should do to check stuff first? (Can't figure out how to upload Dr Prius screenshots, PriusChat just keeps saying there was a problem uploading it, .jpg files)

    Trouble codes P3021 & P3022 indicate that there is a problem with blocks 11 & 12.

How long do you plan on keeping this car? If it is less than two years, I would try replacing battery modules to keep it going. If more than two years, I would buy a new OEM battery from a local dealership and install it myself.

I aas planning on getting a new car either this year or next. I really don't want to get a new battery - it'd probably cost more than the car is worth at this point.

Well, as an update, the battery just fully failed. Voltages are still the same as before, internal resistance are now all the same at 19, but the "State of Charge" suddenly and quickly dropped from almost 100% to 0% and was stuck there unable to charge. After turning the car off and on again, it did go into charging mode while parked, but does not appear to be doing anything.
