Sometimes it is one of the many computer chips.

Sometimes it is as easy as cleaning out corrosion...

You need to read the error codes that point to the problem!

How's the 12v battery? How old is it? What's the voltage?

I’ll be OBDII testing before driving to the dealer.

You know, my first Gen 2 Prius would give me the incongruous “parking pawl engaged, move to level ground “ message, so I’m hoping when this over 6 year old battery started to fail, the Gen 3 computer freaks out as I just experienced.

Are you using a simple OBDII adapter?
It may not show the source of the problem.

About the only "is this always ... ?" answer I can confidently give is that it's always going to be related to whatever the power management control ECU trouble codes turn out to tell you.

UPDATE! At least in the USA, Toyota resolved a class action suit with a non-recall warranty coverage 15 year extension to 2035 for 2010-2015 Prius and 2013-2015 Prius V, serviced "on demand" if/when the : Hybrid System "Intelligent Power Module" fails, putting Hybrid drive line in fail-safe mode, and lighting up the dash AS I had IMAGED. Free replacement of this remove-able module, it is inside the Inverter casing, which then also gets that cooling fluid drained and replaced free. Diagnostic fees also waived. TEMPORARY REPLACEMENT ride (free loaner or cheap rental) VEHICLE IS NOT, repeat not, COVERED. Oh well, I was hoping. While ETA for this command module is unk at this time, Service mgr national search shows my need is one of just a handful in the country, so he's hopeful the unit is in stock in Corporate warehouse somewhere in the country and will arrive quickly, but details currently UNK, it might not even be in the USA. Basically, this unit notes a non-standard(higher) than normal voltage that could damage other hybrid components, including the drive battery, and shuts the module off, disabling all the hybrid parts. If other modules in the inverter are damaged by that voltage or heat, those are also free replacement. Recall notice attached, but remember, it is ON CONDITION of a failure, no preemptive replacements, only of failures. I got lucky with a reputable Toyota branded dealer, anyone else seeing this could easily lie to customers, say it was a drive battery or complete inverter failure, take it in trade for dirt cheap, then do free repairs and resell it for pure profit. But not this time. Feeling lucky, and until that's done, I'm getting a reasonably priced oil and filter change, tires rotated, and likely a failing rear wheel bearing rumble found and solved while waiting for the IPM board to arrive.

As the only way of knowing whether the problem causing the warning lights in your car is the same problem that service campaign covers or not is to read the trouble codes, I take it from your post above that you took the car in and they read the codes and matched them up to the campaign?

Yes, they told me about the class action resolution;I love an extension to 2035, that was a surprise. And clearly enough folks had this failure that Toyota made this extension, as in states like mine using the CARB pollution standards, the drive battery and hybrid system are warranted to 10 years, 150,000 miles. I'm at 158,000, but it is still, now, a warranted repair. Yeehaw!

Yes, the specific failures that 23TE02 and 23TE03 cover happened to enough cars that Toyota has those two campaigns to cover them.

It's still the case that the warning lights you had come on for a couple hundred different reasons; that's why it is essential to read the trouble codes to know why those lights have come on. If the trouble codes indicate the specific conditions for the 23TE02 and 23TE03 coverage, then that coverage will apply.

Did you hear anything prior to getting this message? Some people reported hearing a "bang" when the IPM or transistors go.

Also, how's your brake booster/pump? I've been driving around with a brake booster that's been recharging the system with increasing frequency, so far no codes. I'm approaching 150K on the odometer (2011 Prius three).

That failure can come with varying amounts of drama. If you look at the technician instructions that go with those service campaigns ... sometimes the tech will just replace the IPM. Sometimes it'll be the IPM and one or two nearby modules. If a hole got blown in the inverter case, you get a shiny new unit.

The rental car is not covered in Toyota's 15 year warranty extension but IS covered in the Class Action Settlement on the inverter. Some sources suggest you had to own the car as of May 2022. This was clarified later to include subsequent purchasers.
