They are not for sale anywhere. I made one for myself and a friend. The jack was only lifted just to get the rear tire up. It can really lift the rear corner up to 16-18 inches at the pinch rails. For the front, a simple one inch high thick wall "locking ring" can be used to lift the two front wheels off the ground with the same jack. That ring is for sale but not here. several have used this template; to make jack adapters (metal as well as hard plastics) for Tesla's 3 locater holes on the X & S. if you do it wrong ... it will do damage, & then you live with regret. rather than do the machine work - we waited until folks started doing the grunt work & putting 'em on ebay but most cars that need something special / proprietary will quickly get produced by end users. .
