Was a NEW actuator installed or a used one? I've seen used ones installed that are bad, many times they will not do the linear valve learning, which is a required step in the replacement procedure.
I've seen several cars with that learning code that will often have a wheel lockup as you're attempting to slow down. In my experience on test drives, it's always a front wheel. Really BAD on wet roads. Can be a scary moment in life. BIG pucker factor.

I've never had a new one fail to work 100% properly.

PS....I think you should probably replace your ABS relays. If one is stuck shut, the other is probably worth replacing also. That relay circuit code could also be wiring or computer, but relays are cheap and most likely.

You had a used actuator install and you can't learn the linear valve offset very common you need another used actuator or a 1400 or so new one I've had to change one on one of my cars twice to get a good un it do happen . I keep a few backed up here usually
