How does the price of Toyota parts & dealer labor compare to other places? Are hybrids excluded as in some of their other offers? The 6 month expiration date on the card is pretty short.
Unless you know how to do this yourself, I suggest that you only take it to a Toyota stealership! Not to many other places know how to work on these type of brakes and do it properly.
