At 200k I decided to make some little changes

    Cosmetic delights are fine, but what maintenance operations on the engine, hybrid installation have you done during this mileage?
    Have the problem with the cylinder head leak, EGR cleaning, hybrid installation maintenance been solved?

    Nothing really just stay up to date with maintenance all from Toyota … even keeping records and receipts… so far no issues

    It'd be an easy foray to check the condition of the interior of this pipe:

    Here's a video on accessing it:



    ^Not that hard, an hour or two easy wrenching. It'll give "some" idea of the rest of the EGR system. The EGR cooler (largish stainless steel item immediately at the exhaust manifold), and the small EGR passages in the intake manifold.

    Proper operation of the EGR (Exhaust Gas dRecirulation) system is arguably crucial for head gasket integrity.

    Looking good! What roof rack is that? Is it permanently installed? It doesn't look like the type that hooks to the door openings.
