The Gen 3 Prius takes an H11 bulb in the oem foglight and the lowbeam headlight (non LED equipped models). The 9005 is used in the highbeam.
Optical disks read from the center of the disk first, so make sure the disk surface at the center portion is clean and scratch free.
That is the lowbeam headlight. The highbeam is the open reflector next to it, which should also be on when in highbeam mode. The H11 lowbeam...
