It is very obvious that one of the two modules that make up block #12 is bad. You need to replace that module OR replace the complete battery...
The extended warranty was for 150,000 miles OR 10 years after delivery of the car, whichever comes first. So, your 2012 is past the 10-year limit.

You need to buy a Prius capable scan tool to read ALL the trouble codes. See post #2 and #3.

Double check that ALL the vacuum hoses are connected properly. Also, check to make sure that the air inlet assembly was put back correctly.
