Following the steps I outlined in an earlier post, I was able to pair my Treo 650 and my Prius reliably. Now I have a new bluetooth headset, a Plantronics Discovery 655 if you're wondering. Do I need to turn off that headset before starting the car? I could try it and see, but I don't receive or make that many calls, in the car or elsewhere.

So if you had it connected with the earpiece, it won't connect to your car until you unconnect the earpiece. And vice versa. You don't have to turn it off, just deconnect. But turning it off is much easier.
